Primary Head Replacement for Intertrochanteric Fractures of the Femur


Abstract
The authors report 30 cases of intertrochanteric fractures of the femur that had been treated primarily by prosthetic replacement of femoral head during the period of 10 years between 1979 and 1988. The purpose of this paper is to. review our experience in an attempt to delineate the pri mary indication of the prosthetic replacement in inteitrochanteric fracture of the femur. The results were as follows; 1. Thirty intertrochanteric fractures were comprised of 19 stable and 11 unstable fractures. 2. The prostheses inserted were metal head in 15 hips and ceramic head in 15 hips. 3. There were no serious early postoperative complications except one death due to associated systemic disease. The late postoperative complications were 4 acetabular erosions, 4 heterotopic ossifications. 2 prosthesis sinkings and one loosening. 4. The hip function of seventeen hips followed more than one year showed excellent and good in 10, fair in 5, and poor in 2. Based upon these results, prosthetic head replacement may be recommended for the elderly patient with a intertrochanteric fracture for the rapid restoration to prefracture state.
